    KETOgenic diet therapy in patients with HEPatocellular adenoma:Study protocol of a matched interventional cohort study

    INTRODUCTION: Hepatocellular adenoma (HCA) is an uncommon, solid and benign liver lesion, mainly occurring in women using oral contraceptives. Patients are advised to stop using oral contraceptives (OC) and, as overweight is frequently observed, dietary restrictions. Metabolic changes are assumed to play a role and it has been suggested that diet may help to reduce tumour size. A low-calorie ketogenic diet (LCKD) has been shown to induce weight loss and multiple metabolic changes, including the reduction of portal insulin concentrations, which downregulates hepatic growth hormone receptors. Weight reduction and an LCKD can potentially reduce the size of HCAs. METHODS AND ANALYSIS: We designed a matched, interventional cohort study to determine the effect of an LCKD on the regression of HCA. The study population consists of female subjects with an HCA, 18–50 years of age, body mass index>25 kg/m(2), who are entering a surveillance period including cessation of OC. A historical control group will be matched. The intervention consists of an LCKD (approximately 35 g carbohydrate/1500 kcal/day) for 3 months, followed by a less strict LCKD for 3 months (approximately 60 g carbohydrate/1500 kcal/day). Main study endpoint is the diameter of the HCA after 6 months, as compared with the historic control group. Secondary endpoints include adherence, quality of life, change in physical activity, liver fat content, body weight, body composition and resting energy expenditure.     Research in global change ecology relies heavily on global climatic grids derived from estimates of air temperature in open areas at around 2 m above the ground. These climatic grids do not reflect conditions below vegetation canopies and near the ground surface, where critical ecosystem functions occur and most terrestrial species reside. Here, we provide global maps of soil temperature and bioclimatic variables at a 1-km2 resolution for 0–5 and 5–15 cm soil depth. These maps were created by calculating the difference (i.e. offset) between in situ soil temperature measurements, based on time series from over 1200 1-km2 pixels (summarized from 8519 unique temperature sensors) across all the world\u27s major terrestrial biomes, and coarse-grained air temperature estimates from ERA5-Land (an atmospheric reanalysis by the European Centre for Medium-Range Weather Forecasts). We show that mean annual soil temperature differs markedly from the corresponding gridded air temperature, by up to 10°C (mean = 3.0 ± 2.1°C), with substantial variation across biomes and seasons. Over the year, soils in cold and/or dry biomes are substantially warmer (+3.6 ± 2.3°C) than gridded air temperature, whereas soils in warm and humid environments are on average slightly cooler (−0.7 ± 2.3°C). The observed substantial and biome-specific offsets emphasize that the projected impacts of climate and climate change on near-surface biodiversity and ecosystem functioning are inaccurately assessed when air rather than soil temperature is used, especially in cold environments. The global soil-related bioclimatic variables provided here are an important step forward for any application in ecology and related disciplines. Nevertheless, we highlight the need to fill remaining geographic gaps by collecting more in situ measurements of microclimate conditions to further enhance the spatiotemporal resolution of global soil temperature products for ecological applications

    Atrasentan and renal events in patients with type 2 diabetes and chronic kidney disease (SONAR): a double-blind, randomised, placebo-controlled trial

    Background: Short-term treatment for people with type 2 diabetes using a low dose of the selective endothelin A receptor antagonist atrasentan reduces albuminuria without causing significant sodium retention. We report the long-term effects of treatment with atrasentan on major renal outcomes. Methods: We did this double-blind, randomised, placebo-controlled trial at 689 sites in 41 countries. We enrolled adults aged 18–85 years with type 2 diabetes, estimated glomerular filtration rate (eGFR)25–75 mL/min per 1·73 m 2 of body surface area, and a urine albumin-to-creatinine ratio (UACR)of 300–5000 mg/g who had received maximum labelled or tolerated renin–angiotensin system inhibition for at least 4 weeks. Participants were given atrasentan 0·75 mg orally daily during an enrichment period before random group assignment. Those with a UACR decrease of at least 30% with no substantial fluid retention during the enrichment period (responders)were included in the double-blind treatment period. Responders were randomly assigned to receive either atrasentan 0·75 mg orally daily or placebo. All patients and investigators were masked to treatment assignment. The primary endpoint was a composite of doubling of serum creatinine (sustained for ≄30 days)or end-stage kidney disease (eGFR <15 mL/min per 1·73 m 2 sustained for ≄90 days, chronic dialysis for ≄90 days, kidney transplantation, or death from kidney failure)in the intention-to-treat population of all responders. Safety was assessed in all patients who received at least one dose of their assigned study treatment. The study is registered with ClinicalTrials.gov, number NCT01858532. Findings: Between May 17, 2013, and July 13, 2017, 11 087 patients were screened; 5117 entered the enrichment period, and 4711 completed the enrichment period. Of these, 2648 patients were responders and were randomly assigned to the atrasentan group (n=1325)or placebo group (n=1323). Median follow-up was 2·2 years (IQR 1·4–2·9). 79 (6·0%)of 1325 patients in the atrasentan group and 105 (7·9%)of 1323 in the placebo group had a primary composite renal endpoint event (hazard ratio [HR]0·65 [95% CI 0·49–0·88]; p=0·0047). Fluid retention and anaemia adverse events, which have been previously attributed to endothelin receptor antagonists, were more frequent in the atrasentan group than in the placebo group. Hospital admission for heart failure occurred in 47 (3·5%)of 1325 patients in the atrasentan group and 34 (2·6%)of 1323 patients in the placebo group (HR 1·33 [95% CI 0·85–2·07]; p=0·208). 58 (4·4%)patients in the atrasentan group and 52 (3·9%)in the placebo group died (HR 1·09 [95% CI 0·75–1·59]; p=0·65). Interpretation: Atrasentan reduced the risk of renal events in patients with diabetes and chronic kidney disease who were selected to optimise efficacy and safety. These data support a potential role for selective endothelin receptor antagonists in protecting renal function in patients with type 2 diabetes at high risk of developing end-stage kidney disease.     Het Sociaal Fonds voor de PrivĂ©-Ziekenhuizen gaf in 1996 de opdracht aan het Hoger Instituut voor de Arbeid van de K.U.Leuven en aan de Dienst Socio-Economie de la SantĂ© van de UCL om de werkgelegenheid van de zorgberoepen in respectievelijk de Vlaamse en Franse Gemeenschap in kaart te brengen. Uitgangspunt was de vraagstelling welke de huidige werkgelegenheid is in de zorgsectoren, voor welke kwalificaties, en welk de toekomstperspectieven zijn. Hiervoor werd een gedetailleerde analyse gemaakt van de vraag en het aanbod in de betrokken sectoren en beroepen. Welke evolutie is er in de diverse voorzieningen (toenemende vraag, wijziging in de omkadering) in het recente verleden geweest en wat zijn de verwachtingen, extrapolaties of mogelijke scenario’s voor de toekomst. Hoe evolueerde het aanbod van arbeidskrachten in het recente verleden, en welk zijn de vooruitzichten voor de toekomst (interesse voor de opleiding, instroom van het onderwijssysteem in het beroep, loopbaan van diegenen actief in de sector). Voor de verpleegkundigen kan gesteld worden, dat na een aanzienlijke expansie van de vraag in het verleden en de wijdverspreide opinie dat er schaarste was aan verpleegkundigen, er naar de toekomst toe, bij ongewijzigd beleid, slechts een beperkte expansie zal optreden in de vraag terwijl er een aanzienlijke groei van het aanbod zal optreden. Deze onverwachte vaststellingen en prognoses naar de toekomst nodigen alle actoren rond de tafel uit om het beleid te wijzigen. Voorliggend rapport behandelt de vraag naar en het aanbod van verpleegkundigen en het verplegend werk in de Vlaamse Gemeenschap, uitgevoerd door het HIVA. Voor de Franse en Duitstalige Gemeenschappen is een soortgelijk onderzoek uitgevoerd door het SESA-UCL. Van beide onderzoeksrapporten is een synthese beschikbaar in, zowel het Nederlands als het Frans. Voor de Vlaamse Gemeenschap is er eveneens een tweede rapport voorhanden over de gekwalificeerd verzorgenden en een derde deel (over de overige zorgberoepen) in de zorgsector.nrpages: 308status: publishe
